Please utilize the information below to facilitate the referral to The CORE Institute. If you need assistance during any stage of the referral process, please contact your physician relations coordinator directly or email us at physicianrelations@thecoreinstitute.com.

Other ways you can refer your orthopedic patient:

• • Provide the patient with The CORE Institute contact information and they may schedule an appointment at their convenience 866.974.2673.
  • Click Schedule Online Now for 24/7 accessibility to our online scheduling system.  Please remind the patient to complete the “Referring Physician” field in order for your office to be notified when the patient has been scheduled.
  • Download and complete the CORE Refer form and fax to:

Once a CORE team member receives the fax, we will call the patient to schedule an appointment with the appropriate CORE physician.
